We need a full-time STNA Social Services Designee to join our team in Greater Cincinnati & Northern, KY.
You’ll assisted our Licensed social Worker by ensuring that the medically-related emotional and social needs of our residents are individually met and maintained.
$15-$18.50 Per Hour
You’ll Be Responsible For:
- Assisting with gathering information from residents and family members
- Assisting with the completion of the Integrated Personal Services Assessment and MDS
- Assisting with the discharge planning process by arranging community resources, alternative placement, etc.
- Attending Care Conferences as directed by the LSW
- Assisting LSW with any other case management tasks deemed necessary to the Department.
- Fulfilling basic documentation requirements for social services.
- Assisting (after training) in the BIMS and the PHQ-9
You’ll Need To Have:
- An STNA certification or eligibility and experience in long term care.
- Excellent communication skills
- A High School Diploma and/or GED
- Healthcare industry, social work, or other related experience
- The ability to multi-task in a fast-paced, high volume environment
- Exceptional organization skills with strong attention to detail
- A personable and professional demeanor
Computer proficiency in Excel, Word, Power Point, and Outlook
